Analysis request forms must be filled out for each set of samples shipped to PhycoTech, Inc. If we do not receive an analysis request form for your samples, we may place a hold on processing your samples until all paperwork is received. Please contact us if you have any questions!

Fill Out Form

  1. Download either the excel analysis form or fillable pdf version of the form and save a copy to your computer. We recommend using the Excel version because the conditional formatting and drop-down lists in the 'Sample Details' table will aid you in filling out the form.
  2. Complete the form and save.
  3. Send us your form by completing the following steps:
    • Print and send in the cooler or box with your samples (we suggest putting it in a Ziploc bag)
    • Send electronically by one of the following methods:
